Co-payments and payments of any kind may be collected at the session. Cash, check or charge payments are acceptable.
Please confirm that your insurance company will cover services provided, as having insurance does not guarantee that your insurance company will make payment to Ho`olilo Counseling for the services you request. It is your responsibility to pay for any amount that your insurance policy does not cover as well as general excise tax if your plan permits.
Should you “no call or no show” for a scheduled session, you will be charged a fee of $50.00, which is not covered by your insurance. You are responsible for any fees incurred as a result of missed sessions.
